Health and Human Services Programs are increasingly recognizing the critical role that social determinants play in health outcomes. Social determinants – factors such as safe housing, local food markets, access to educational, economic, and job opportunities, access to health care services, and quality of education and job training – are critical to supporting the physical and behavioral health outcomes of a population. In an effort to bring together programs to improve outcomes, state agencies across the globe are establishing cross-program initiatives aimed at addressing social determinants of health. At this moderated session, attendees will learn about best practices in the US, UK, and Denmark and how governments are addressing elements such as governance, funding, and shared outcomes management for innovative programs.
